# Do I need a training license before residency starts?

In 42 of 51 jurisdictions, yes: you need a training license or permit from the state medical board before you see patients, and the application can take weeks to months. Four states issue no separate training license, and five decide case by case. The risk is timing, not eligibility — start the application after Match Day, not in June.

## The rule

A training license is a limited license the state board issues to residents. It goes by different names — training license, postgraduate permit, intern license, graduate license — and it is separate from a full license to practice unsupervised.

The requirement is state law, not program policy, so it follows the state where you matched. Each state page lists the name of the license, whether it is required, the lead time the board states, the fee, the renewal term, and who has to apply.

Timing is the part that goes wrong. Boards vary in how long they take, and some require the program to submit paperwork first. Confirm the deadline with your program coordinator and apply as soon as the Match agreement is signed.

A full license is a separate question, and it matters mainly for moonlighting. The state page states when you become eligible; some states let you sit for it after one or two years of training.

Your state also sets the wage tax that decides your take-home. The same page carries the rate, and the Match Day plan turns it into a first-year budget.

## Numbers

- Training license required: 42 (of 51 jurisdictions)
- No separate training license: 4 (check program onboarding)
- Depends on program or applicant: 5 (confirm with the board)
- Jurisdictions covered: 51 (checked with each board)

## Actions

- Open your state's page for the license name, lead time and fee. — https://www.joinandwise.com/residency
- Ask your program coordinator for the board's deadline and who submits first.
- Plan the cash for the fee and the move before your first paycheck. — https://www.joinandwise.com/residency/plan
- Check when a full license becomes available if you plan to moonlight.

## What could change this

- Your state is one of the four with no separate training license — the program handles onboarding instead.
- Your specialty or program requires a full license before certain rotations.
- The board changes its lead time or fee — the state page carries the date it was checked.
- You are an international graduate — additional certificate steps may apply before the license.

## When the numbers aren't enough

- Your program says you can start before the license issues.
- Your board has not responded and orientation is weeks away.
- You plan to moonlight and need to know when a full license is available.
